Shoulder Joint Replacement

Total shoulder replacement, also known as total shoulder arthroplasty, is the removal of portions of the shoulder joint, which are replaced with artificial implants to reduce pain and restore range of rotation and mobility. It is very successful for treating the severe pain and stiffness caused by end-stage arthritis.

Shoulder arthritis is a condition in which the smooth cartilage that covers of the bones of the shoulder degenerate or disintegrate. In a healthy shoulder, these cartilage surfaces permit the bones to comfortably glide against one another. When these cartilage surfaces disappear, the bones come into direct contact, increasing friction and causing them to roughen and damage each other. Bone-on bone movement can be quite painful and difficult. Surgically implanted artificial replacement surfaces restore pain-free movement, strength and function.

Hemireplacement

  • It is the replacement of half of the shoulder joint only that is humerus head(ball) with humerus stem.
  • Surgical steps include open the shoulder joint and humeral head is removed. humerus shaft is prepared. Metal stem with ball is inserted and shoulder joint is reduced.
  • Common indication – fracture upper humerus fracture, shoulder arthritis in less active patient
  • Types – cemented and uncemented, modular, unipolar or bipolar

Total Shoulder Arthroplasty

  • It is the replacement of humeral head (ball) and resurfacing of glenoid(socket) to provide pain relief. Surgical steps are same as hemiarthroplasty with additional socket preparation and resurfacing.
  • It has 3 component humerus stem with head (inserted in the humerus) ,glenoid cup (resurface the  socket) and liner component (articulate other two component). Stems and cups are available in different size , length and diameter.
  • Indication – osteoarthritis of shoulder joint, avascular necrosis and inflammatory arthritis. It requires normal cuff muscles to functions.
  • Types – cemented and uncemented
  • Metal(titanium or cobalt-chromium)/ceramic(liner and femur head)/ polyethylene (cup and liner)
  • complication – infection, dislocation or subluxation, nerve or vessel injury, component loosening and fracture.. This can be avoided with proper preoperative planning and postoperative care.

Reverse Shoulder Arthroplasty

  • In a reverse total shoulder replacement, the ball is fixed to the glenoid and the plastic cup is fixed to the upper end of the humerus. It is used in the conditions where rotator cuff muscle is not functioning or torn. 

  • its function mainly depends on the deltoid muscle function. It is used in the shoulder arthritis with cuff muscle torn, arthropathy related to cuff tear and proximal humerus fractures.

  • it is recent advance technology which involves only resurfacing of the humeral head without stem insertion. Commonly used in the shoulder arthritis where onle  humeral head is involved.

  • Total elbow arthroplasty– used in elbow arthritis and comminuted fracture around elbow in older patient. It resurfaces distal humerus and upper ulna with interconnected implant.

  • Total ankle arthroplasty– used in severe ankle arthritis and comminuted fractures in old patient to maintain ankle movement
